These forms must be completed at least 48 hours before your assessment.
Incomplete forms may result in your appointment being rescheduled or cancelled.

Follow-ups and ADHD coaching sessions:48 hours’ notice required
Appointments cancelled with less notice, or missed without notice, are charged at the full fee.
