Pre-assessment requirements
Pre-assessment forms are a required part of your appointment and must be completed in advance.
Your questionnaires will be sent shortly after booking
If you do not receive these within 24 hours, please contact us at support@cognitioncare.co.uk
All forms must be completed and submitted at least 48 hours before your appointment
If forms are not received within this timeframe, your appointment may need to be postponed to ensure a safe and effective assessment.
What happens next
Once your forms have been reviewed, your clinician will be fully prepared for your appointment.
Appointments take place via secure video consultation. Please ensure you are in a private, quiet environment at the time of your session.
Please ensure your contact details are correct and that you check your inbox (including junk/spam folders) for all correspondence.
Changes and cancellations
If you need to change or cancel your appointment, please refer to our cancellation policy.
Appointments require sufficient notice to change or cancel. Where sufficient notice is not provided, the full appointment fee may be charged.
